Booking an appointment is easy.
Options:
- Your referring physician’s
staff calls us and books an appointment for you.
- Your referring physician completes
our referral form and faxes it to us; we call you
to book your appointment.
- If you don't have a referral you
may call us to book an appointment directly. However,
we ask that you arrange for a referral to be faxed
to our offices before your appointment.
Note: a referral is required for your
consultation with our specialists. Without a referral,
you will be charged for a portion of that visit.
